Probable Causes (ranked)

Fix Steps

  1. 1 In RSLogix 5000, check I/O tree for the faulted module — right-click > Properties > General shows fault status
  2. 2 Reseat the module — open retention latches, pull module straight out, re-insert firmly until latches click
  3. 3 Check module status LED — green=OK, red=fault, flashing red=needs update or config error
  4. 4 If input module fault, try disconnecting field wiring — if fault clears, issue is in field wiring (shorted sensor, etc.)
  5. 5 Verify module catalog number matches what is configured in the I/O tree — 1756-IB16 is not 1756-IB32
  6. 6 Update module firmware using ControlFlash if firmware mismatch is indicated
  7. 7 If module is dead after reseat, swap to a different chassis slot to rule out backplane connector issue
